01Receive support, referral, provider, and community inquiries through approved phone, email, website, and secure-channel workflows.
02Create an accurate intake record, confirm contact preferences, and collect only the approved information needed for routing and follow-up.
03Explain Chancewell's process in clear language without promising eligibility, coverage, admission, a specific provider, or a particular start date.
04Verify that required referral and administrative documents have been received, identify missing items, and follow up through approved channels.
05Route clinical records and protected health information only through authorized secure systems and apply minimum-necessary access practices.
06Coordinate scheduling for eligible assessments, clinical interviews, or other approved next steps and communicate preparation instructions.
07Maintain current status, ownership, follow-up dates, contact attempts, outcomes, and closure reasons for every inquiry.
08Recognize statements indicating immediate safety concerns and follow the defined escalation procedure without attempting to provide clinical advice.
09Monitor unresolved inquiries, aging referrals, failed contacts, and access barriers and escalate patterns to the appropriate operational leader.
