Mental Health Case Manager

Provide only authorized mental health targeted case-management activities for assigned individuals in accordance with the plan of care and applicable program requirements.

Assess practical barriers to care and coordinate medical, behavioral, educational, housing, benefits, transportation, and other community resources connected to identified needs.

Create clear referral and follow-up actions, confirm whether connections were completed, and document barriers when a resource was unavailable or declined.

Monitor implementation of the plan of care and communicate meaningful changes in needs, functioning, access, or risk to the appropriate supervisor or clinician.

Coordinate authorized communication with individuals, families, providers, schools, hospitals, managed-care representatives, and community organizations.

Complete field contacts, collateral contacts, coordination notes, and follow-up records accurately and within required documentation timeframes.

Maintain firm boundaries between case management, psychotherapy, skills training, transportation, personal assistance, and other non-covered activities.

Follow field-safety, privacy, incident-reporting, and escalation procedures during home, school, office, and community-based work.

Participate in required supervision, case review, training, competency demonstration, documentation review, and corrective action when indicated.
