Rehabilitation Specialist

Provide authorized, goal-directed rehabilitative and skills-building services that help individuals strengthen functioning, symptom management, independence, and participation in daily life.

What you will own

These responsibilities describe the core work of this role. Final assignments remain within the professional's verified qualifications, scope, privileges, and program requirements.

Deliver authorized, face-to-face or approved remote rehabilitative interventions that directly address goals and objectives in the current plan of care.

Teach, model, rehearse, and reinforce practical skills related to symptom management, emotional regulation, communication, daily functioning, recovery, and community participation.

Adapt each intervention to the individual's age, learning needs, culture, strengths, barriers, and demonstrated response while remaining within the authorized service.

Observe and document the individual's participation, progress, barriers, functional change, and next clinically relevant step rather than recording generic activity summaries.

Engage parents, caregivers, and natural supports when the plan authorizes their involvement and the intervention advances the individual's goals.

Identify changes in symptoms, functioning, risk, or service needs and promptly escalate them through the defined clinical and supervisory pathway.

Coordinate with therapists, case managers, supervisors, and other authorized team members without providing psychotherapy or independently changing the treatment plan.

Follow privacy, field-safety, transportation, incident-reporting, and emergency-escalation procedures in homes, schools, offices, and community settings.

Participate in documented supervision, direct observation, competency review, training, documentation review, and corrective action when indicated.

Minimum qualifications

Verified eligibility for organizational QMHP-CS credentialing or another expressly authorized category.
Required education, training, competency, and supervision documentation.
Ability to deliver services safely in homes, schools, and approved community settings.
Reliable transportation and required driving documentation.
Successful background, exclusion, credentialing, and safety screening.

Preferred qualifications

Mental health rehabilitation, skills training, youth, family, or community experience.
Trauma-informed and person-centered practice.
CANS or ANSA familiarity.
Strong documentation and field-safety skills.
